• Continuum of care is a comprehensive plan of care that adjusts to the needs of the resident throughout their stay.

Why is it Important?

• Cypress Place and Magnolia Place’s continuum of care program will build on any future changes to a resident’s needs from the very beginning. Our trained staff work together to carefully monitor the health of our residents so if changes arise, we can update their level of care quickly to ensure better health outcomes, lower stress, and reduced expenses.

Whether your needs allow for an independent environment with assistance of daily living at Cypress Place,  or your needs shift to requiring more around-the-clock care in the form of rehab-to-home, long-term care, skilled nursing, and/or secured memory care at Magnolia Place, our continuum of care program can respond effectively to preserve and expand the highest quality of life and independence for each resident.
